Midwestern Dental Has Ceased Operations in Michigan

MAGE has been informed by the State of Michigan that Midwestern Dental will be ceasing all operations in Michigan. If you obtained coverage through Midwestern Dental, below is the email notification that you should have received from the State of Michigan. If you have any questions, please be sure to contact the MI-HR Service Center.
 
"Midwestern Dental has informed the state that they will permanently cease operations and close all locations this week. As a result, Midwestern Dental will no longer offer dental insurance benefits to you after May 29, 2020.

This was unexpected, and we understand the frustration and difficulties it may cause. To ensure continuity of coverage, you will be automatically enrolled in the State Dental Plan, through Delta Dental, , effective May 30, 2020.
 
If you wish to continue coverage in the State Dental Plan, no further action is required. If you instead wish to enroll in the State Preventive Dental Plan or waive coverage, you must contact the MI HR Service Center by June 30, 2020, to request that change. If you do not contact the MI HR Service Center (877-766-6447, Monday - Friday, 8:00 a.m.-5:00 p.m.) by this deadline, you will continue with coverage in the State Dental Plan until your next opportunity to make a change during the upcoming open enrollment period or another qualifying life event occurs.
 
You will receive a letter from Midwestern Dental with instructions on how to obtain your dental records and where to direct any billing related questions you may have.
